
When we hear the word “donation,” most of us think of blood, kidneys, or eyes. But did you know you can also donate your brain? Unlike other organs, a donated brain cannot be transplanted into another person. Instead, it becomes a powerful gift for science. Researchers study donated brains to understand diseases like Alzheimer’s, Parkinson’s, schizophrenia, and depression.
In India, brain-related illnesses are increasing. By 2050, nearly 1.1 Crore Indians are projected to live with dementia, most commonly Alzheimer’s disease (Shaji et al., 2018). Yet, our country has very few brain samples available for research. Most of the knowledge we rely on comes from Western countries, where lifestyle, genetics, and environment are very different from ours. This creates a major gap in treatment and understanding.
Brain donation is one way to bridge this gap. Every donated brain can help doctors and scientists develop better ways to detect illness early, improve treatments, and even reduce stigma around mental and neurological disorders. Still, awareness about this option remains very low in India.
What is Brain Donation?
Brain donation is the process of giving your brain to science after death. Unlike organ donation, where hearts, kidneys, or eyes are used to save another person’s life, donated brains are used for research and medical education. This means your brain will never be transplanted, but it will help doctors and scientists study illnesses more closely.
After a person passes away, with the consent of their family, doctors carefully remove the brain in a respectful and dignified manner. The body is then handed back to the family, so traditional funeral, burial, or cremation rituals can continue without interruption. The donated brain is preserved in a brain bank, a facility designed to store and protect brain tissue for research.
Both healthy and diseased brains are valuable. Healthy brains help researchers understand what normal aging looks like. Diseased brains such as those affected by Alzheimer’s, Parkinson’s, or schizophrenia, help scientists see exactly how illness changes brain cells and connections. By comparing the two, researchers can discover ways to diagnose earlier, treat better, and maybe even prevent such conditions in the future (Pandya et al., 2019).
In India, very few people know about this possibility. At present, only a handful of brain banks exist, including those at NIMHANS (Bengaluru) and AIIMS (New Delhi). Every donated brain becomes a rare and precious resource for Indian science.
Why is Brain Donation Essential?

With an ageing population, conditions like dementia and Alzheimer’s are expected to rise sharply. By 2050, almost 1.5 crore Indians may be living with dementia (Shaji et al., 2020). At the same time, millions struggle with psychiatric conditions like schizophrenia, depression, and addiction.
Despite this, most of the research guiding treatment in India comes from Western countries. But brain diseases don’t always look the same everywhere. Our genes, diet, lifestyle, and cultural factors make the Indian population unique. For example, research on Alzheimer’s disease in the West may not fully capture how the illness progresses in Indian patients (Alladi et al., 2020). Without Indian brain samples to study, doctors and scientists are left with incomplete knowledge.
This gap is where brain donation becomes so important. Every donated brain can help researchers:
- Understand how diseases affect Indian brains.
- Develop diagnostic tools suited to our population.
- Create treatments that are more effective and affordable.
- Reduce stigma around mental and neurological illness by showing these are medical conditions, not moral failings.
At present, India has only a few active brain banks, such as those at NIMHANS and AIIMS, yet the demand for samples is far greater than the supply. Building a culture of awareness around brain donation could change the future of neuroscience and psychiatry in India.
Current Situation
At present, India has only a handful of brain banks. The first was set up at NIMHANS, Bengaluru, in 1995, followed later by others at AIIMS, New Delhi. Together, these centers have made important contributions to research on dementia and psychiatric illnesses. However, compared to the size of our population, the number of donated brains remains very small.
Several challenges stand in the way:
- Low public awareness: Most families have never heard of brain donation until approached by doctors.
- Cultural discomfort: Talking about death and body donation is often seen as taboo.
- Religious concerns: Families may hesitate, worrying that donation might interfere with rituals.
- Limited infrastructure: Only a few cities have facilities to retrieve and preserve donated brains quickly. This makes it difficult for people in rural or smaller towns to participate.
- Need for family consent: Even if a person pledges to donate, the family must agree at the time of death, which can be a stressful moment.
Because of these barriers, India continues to rely heavily on Western research. But without more local brain donations, scientists here cannot fully understand how diseases affect Indian brains. Expanding awareness and infrastructure is essential to close this gap.
Stories and Initiatives From India
While brain donation is still rare in India, a few courageous families and institutions have taken steps to change the narrative. These stories remind us that real progress is possible.
In February 2024, the brain bank at NIMHANS (Bengaluru) received its first brain from a patient with Huntington’s disease. The donor was a 62-year-old woman whose son, an engineer, approached NIMHANS to fulfill her wish.
The procedure was done late at night, and the team worked past midnight to retrieve and preserve the brain. This donation marks a milestone: for the first time in India, researchers now have brain tissue from an HD patient to study. Before this, studies relied mostly on blood samples or animal models, which only tell part of the story.
The NIMHANS neuropathology team noted that such donations help deepen understanding of how the disease works in the human brain and may pave the way for better therapies.
NIMHANS has had a brain bank since 1995. Over the years, it has collected brains mostly from patients with dementia and neurodegenerative disorders. However, the number of donations is still extremely small compared to demand. Many cases occur in remote areas, and families may not know about the option or may miss the short time window needed for retrieval.
In recent years, awareness campaigns by neurological societies, medical colleges, and NGOs have begun to include brain donation in their outreach, especially in the fields of dementia and Alzheimer’s research. While published, large-scale data is still sparse, these efforts are essential first steps.
How Brain Donation Helps Science and Society
Donating a brain gives scientists the chance to directly study how illnesses affect nerve cells and brain tissue. This is vital for conditions like Alzheimer’s, Parkinson’s, schizophrenia, and depression, where blood tests or scans alone cannot provide full answers.

Myth 1: Brain donation disfigures the body.
Fact: The procedure is done respectfully by trained doctors. The face and body are not affected, and families can still carry out traditional funeral, cremation, or burial rituals without interruption (Pandya et al., 2019).
Myth 2: Only healthy brains are useful.
Fact: Brains with illnesses like Alzheimer’s, Parkinson’s, or schizophrenia are just as valuable, sometimes even more because they help scientists understand how diseases progress. Healthy brains are used for comparison.
Myth 3: Religion does not allow brain donation.
Fact: Most major religions in India, including Hinduism, Islam, Christianity, Sikhism, and Buddhism, support organ and tissue donation as an act of service and compassion (Shroff, 2009). Brain donation is no different.
Myth 4: Families cannot consent if the person has not pledged.
Fact: Even if someone has not registered as a donor, their family can still give consent after death. This makes talking to family members in advance very important.
Myth 5: Brain donation is only for scientists abroad.
Fact: India has its own brain banks, such as those at NIMHANS and AIIMS, where donated brains are studied to improve treatments for Indian patients.
How Can You Pledge to Donate Your Brain in India?
Pledging to donate your brain is possible in India, though not many people know how the process works. Here are the main steps you and your family should be aware of:
Talk to Your Family First
Brain donation requires the consent of your next-of-kin at the time of death. Having a prior discussion with your family is the most important step. Let your caregivers and doctors know about your decision as well.
Register With a Brain Bank
You can contact brain banks such as the Human Brain Tissue Repository (HBTR) at NIMHANS, Bengaluru or AIIMS, New Delhi, and fill out a pledge form. Some banks issue a donor card that you can carry.
Call Immediately at the Time of Death
Since brain tissue needs to be preserved quickly, family members should call the brain bank staff as soon as death occurs or is imminent. The team will guide the family through the next steps. Ideally, the brain should be retrieved within 4 to 6 hours after death for the best quality tissue. The shorter the gap, the better the quality of samples for research. Some centers accept donations up to 48 hours after death.
Provide Essential Documents
Families will be asked for:
- Photo ID proof of the deceased
- Death certificate (original)
- Contact details of the legal next-of-kin
- Medical records if available
- Donor card, if the person had registered
Legal Consent and Retrieval
A consent form signed by the legal next-of-kin is required. The hierarchy of who can give consent starts with the spouse, followed by adult children, parents, siblings, and so on. The retrieval is done respectfully, usually within 1–1.5 hours at the hospital mortuary. The body is then returned to the family for final rites.
Acknowledgement
Families receive an acknowledgment, a certificate of donation, and a thank-you letter from the brain bank. This serves as a record of their loved one’s contribution to science.
By knowing these steps in advance, families can avoid last-minute confusion and ensure that the donor’s wishes are honored. Every donation not only supports research but also becomes a legacy of service.
